How to Upload Your Records:

The Medical Records Upload Process takes only 4 simple steps. Please read through the entire 4-step instructions before completing the form below. These steps ensure your records are secure and protected.

STEP 1. Begin Upload Process

After filling the following blank fields, please click on: A Personalized Email should arrive in your email inbox.

STEP 2. Open Email from: Cedars-Sinai SHI Takotsubo Registry

The email contains information from . It was created and sent only to you using the information you provided in STEP 1. Click on:

STEP 3. Create your dedicated Box Folder

After completing STEP 2 you will receive a prompt inviting you to label your new Box Folder.

Once your information has been entered, click on:



STEP 4. Upload your files

Your medical records can be now be uploaded to the folder you just created. You can simply highlight your files/folders and drag the records from your CD (or file folder on your computer) to the Folder.

If you have a slow internet connection (less than 100Mbps) then your computer may time-out before fully uploading all the files. If this happens you may either perform a partial upload (select 10%-15% of your files) each time and upload one section at a time. Alternatively, you can mail a copy of your CD/DVD to:
